15-20% the HBV of those who have HBeAG - and anti-HBe + can be reactivated suddenly (and cause death)

Anti-HBe-positive patients may have much lower viral loads, which may be undetectable in blood by standard assays. These patients usually have little ongoing liver damage. Anti-HBe-positive patients may be infected with the so-called “pre-core” mutant, which does not produce HBeAg. These patients may have detectable HBV DNA and may develop progressive liver disease leading to cirrhosis, and therefore merit life-long observation.

Even in healthy immunocompetent adults, about 5% will not develop protective antibodies after a course of vaccination. ..... If a second complete course of vaccinations fails to induce protective titres, you will have to sadly accept that you are not protected against hepatitis B.
